Muscle mass loss that may accompany weight loss with medicines is a significant concern. It is critical to understand that the muscle mass loss is almost completely correlated to the pace of weight loss and not the medicine itself. As such, we need to make sure that we have realistic expectations, and understand that it is unwise to lose faster than a certain pace. The pace of weight loss that is recommended, not only because of the sustainability (by reducing the significant increase in hunger and inability to maintain the lifestyle changes you make by more rapid weight loss), is 0.5 to 2.0 pounds per week. This correlates to 2.15-8.6 pounds per month, which is almost always below the hopes and unrealistic expectations people set because "their friend" or "the people on TV" are having greater success. If you start to lose faster than this, we need to actually try to slow you down, by adjusting the medicine. This is especially important at the beginning, when the fastest weight loss always occurs. Increasing your protein intake, primarily with plant-based protein (and fatty seafood, or a little lean meat for those who like meat), combined with resistance exercises, will also help reduce lean muscle mass loss. We are confident that understanding these principles and acting accordingly will significantly reduce the risk for lean muscle mass loss and improve your chances of long-term weight loss success!
